Sarasota Memorial Health Care System is seeking a dedicated coordinator for a disease-specific program to plan, direct, and manage clinical activities and care delivery. You will maintain program data, liaise with nursing, medical, pharmacy staff and families, and work with leadership to assess needs and implement educational interventions to improve patient safety and outcomes.

Disease Specific Programs
Plans, directs, and manages the clinical activities of a specialty program, and provides specialty care either directly or indirectly to patients. Maintains and monitors the program database and analyzes the data. Serves as a liaison between nursing staff, medical staff, pharmacy staff and other healthcare professionals, patients and their families. Works collaboratively with leadership teams to conduct needs assessments/gap analyses to identify interventions to address gaps in performance. The coordinator develops, plans, and implements education that facilitates the professional growth, skill development, initial/ongoing competence, and attainment of standards of care to enhance patient safety for direct care providers. They evaluate outcomes of interventions, with a focus on quality, safety, and evidence-based practices. The coordinator role requires a collaborative relationship with the leadership teams to implement changes and establish clinical accountabilities for program success.
BSN: Bachelor of Science in Nursing
FL RN: FL Registered Nurse License
