Approves new drug orders, which may include checking/verifying chemotherapy notes, informed consent and processing non-formulary requests. Screens for drug-drug, drug-herb/dietary supplement, drug-allergy, drug-food (in cooperation with the Dietetics Department), drug-disease state and drug-laboratory interactions. Participates in appropriate hospital and/or departmental committees relevant to practice area. POSITION SUMMARY This position functions to coordinate all aspects of patients' pharmaceutical needs in the patient care areas. Duties include but are not limited to: medication order approval and distribution; assurance of appropriate dose, route, frequency and duration of therapy; patient medication profile review for drug-drug interactions, duplication of therapy, drug allergy interactions and appropriate regimens; coordination of patients' pharmaceutical needs with physicians and nurses. Patient population includes adult, adolescent, geriatric, and neonatal patients depending on the practice location. Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. Must be able to demonstrate medication dosing and identify pharmaceutical needs relative to the disease process and age. This position is also responsible for supervising Pharm.D. students, technicians, undergraduate and graduate student teaching, and providing educational programs for hospital staff (physicians, nurses and pharmacy staff). Minimum Education Required Master's Level Degree or equivalent combination of education and experience Required Qualifications Registered pharmacist in the State of Ohio; board certified; advanced degree (Pharm.D./MS) and residency; and/or equivalent experience preferred (e.g. PGY1 and PGY2 residency).## Additional Information:## Location:University Hospital - Doan Hall (0089)## Position Type:Intermittent## Scheduled Hours:0.2## **Shift:**First Shift***Final candidates are subject to successful completion of a background check.
