Job summary

Sentara Health is seeking a Pharmacy Navigator to guide specialty pharmacy patients through their medication journeys in Chesapeake, VA. You will coordinate refills, monitor adherence, and assist with insurance and financial challenges, working closely with pharmacists and clinicians.

Responsibilities include patient education, collaboration with care teams, handling special storage needs, and documenting all activities in required systems.

Qualifications

• Pharmacy Technician license valid in state(s) of practice.
• Minimum 2 years of pharmacy technician experience.

Responsibilities

• Deliver exceptional, personalized care to specialty pharmacy patients, providing a full suite of medication management services.
• Collaborate with clinic and ambulatory care teams, supporting patients in-person and over the phone.
• Educate patients on benefits of the specialty pharmacy and its impact on care outcomes.
• Support outpatient retail and specialty prescription needs, including refills and adherence.
• Monitor patients' medication regimens through outbound check-ins and coordinate timely refills.
• Identify medications requiring special handling or storage (hazardous or refrigerated).
• Respond to and triage inbound patient and provider inquiries.
• Address insurance-related issues including prior authorizations and financial assistance programs.
• Maintain accurate documentation in required systems and data tracking.
• Manage patient data using relevant database applications.
• Apply insights to improve workflow, productivity, and service quality.
• Perform other duties as assigned.
