Job summary

AndHealth, a community-focused health startup, is seeking a Specialty Pharmacist to join our Ohio team at a community health center. You will deliver direct patient care, review profiles, and help secure patient access through payor formularies and prior authorizations.

The role requires a PharmD or equivalent, Ohio license (and willingness to obtain licenses in other states), 3+ years of direct patient care, and strong communication and detail orientation.

Qualifications

• PharmD, BS in Pharmacy, or NABP FPGEC equivalent.
• Unrestricted pharmacist license in Ohio; ability to obtain licenses in multiple states.
• 3+ years of direct patient care in pharmacy preferred.
• BLS/CPR required or willing to obtain within 1 month after starting.

Responsibilities

• Deliver direct care, therapeutic recommendations, and counseling for patients with chronic conditions via the Patient Management Program, protocols, or collaborative practice agreements.
• Review patient profiles for interactions, allergies, or duplications, and recommend evidence-based medication or dosage adjustments.
• Partner with Patient Engagement Specialists to secure patient access by managing payor formularies, prior authorizations, and appeals.
• Maintain strict documentation for URAC and ACHC accreditation and actively participate in Quality Management programs and improvement projects.
• Accurately process, fill, verify, and safely dispense prescriptions while balancing workflows.
• Maintain flexibility to provide on-site support or cross-site remote assistance, ensuring operational continuity as business needs evolve.
