Job summary

Qualifications

• Bachelor’s Degree in Pharmacy.
• Kentucky Board of Pharmacy license in good standing.
• 1-3 years related work experience or completion of ASHP accredited pharmacy residency program preferred.

Responsibilities

• Serve as primary point of contact with patients related to specialty medication needs and provide empathetic counseling.
• Enroll specialty pharmacy patients into medication management programs by assisting recruitment in clinic.
• Provide initial clinical medication counseling and regular patient reassessments via in-person, telephonic, or video encounters.
• Perform medication therapy management and coordinate medication adjustments to optimize regimens and outcomes.
• Coordinate drug distribution in accordance with scope of practice, policies and procedures.
• Collaborate with providers and clinical staff to integrate pharmacy services and coordinate care for eligible patients.
