Job summary

Deaconess is seeking a compassionate Specialty Pharmacist to join our Specialty Pharmacy team in Indiana. You will manage complex therapies, educate patients, and collaborate with clinicians to optimize outcomes in a highly clinical setting.

The role emphasizes patient-centered care, medication management, and ongoing improvement initiatives within a hybrid work model, supporting professional growth and clinical excellence.

Qualifications

• Must hold a BSPharm or PharmD from an accredited school.
• Licensed or eligible for Indiana and Kentucky Pharmacist licensure.
• Ability to obtain licensure within 45 days of employment.
• Strong clinical knowledge and patient-centered care commitment.

Responsibilities

• Manage patient care through the Specialty Pharmacy Patient Management Program.
• Review charts and therapy plans to support safe, effective treatment.
• Educate patients on specialty meds, administration, side effects, and adherence.
• Monitor progress and perform clinical interventions with care teams.
• Identify opportunities to optimize therapy and outcomes.
• Collaborate with physicians, nurses, and other professionals.
• Ensure compliance with accreditation standards and regulations.
• Participate in quality improvement and operational enhancements.
• Precept students and mentor staff as appropriate.
• Provide guidance to pharmacy technicians in daily operations.
• Contribute to educational initiatives and community health programs.
