Job summary

Deaconess is seeking a skilled Clinical Pharmacist to join our hospital team in the Indiana/Kentucky/Illinois region. The role emphasizes patient-focused pharmacotherapy, medication safety, education, and collaboration across disciplines within a 24/7 pharmacy service.

Eligible candidates must hold a BS or PharmD, have licensure in IN/KY/IL or be eligible, and bring 1 year of hospital or specialty pharmacy experience. CSP preferred and sign-on bonus offered.

Qualifications

• Requires a BS or PharmD degree from an accredited school of pharmacy.
• Minimum 1 year hospital or Specialty pharmacy experience; 3 years preferred.
• Pharmacy Practice Residency preferred.
• Experience with direct supervision preferred.
• Licensed or eligible for IN, KY and IL licensure; licensure within 45 days.
• Advanced certification (CSP) within 6 months of hire.
• No criminal records or actions with any Board of Pharmacy.

Responsibilities

• Ensures compliance with regulatory and accreditation standards and maintains policies and procedures.
• Contributes to quality improvement by participating on teams and identifying improvements.
• Designs and monitors pharmacotherapy; provides education, dosing, anticoagulation, rounds with teams.
• Prepares and dispenses medications per policies; maintains formulary and inventory records.
• Develops educational materials; leads staff training and competency programs.
• Acts as a preceptor and trains new staff; oversees programs in coordination with leadership.
• Supports financial development and oversight of daily operations; oversees specialty functions.
