Responsibilities

• Coordinate home health nursing care for patients within a designated specialty caseload.
• Assess patients' physical, social, and environmental needs to inform individualized care planning.
• Develop, implement, and continuously update care plans based on patient status and outcomes.
• Provide direct skilled nursing care in the home setting.
• Document clinical assessments, care provided, and changes in condition accurately and consistently.
• Communicate effectively with physicians, patients' families, and interdisciplinary team members to support treatment continuity and coordinated care.
