Qualifications

  • BSW/BA and at least 2 years related experience.
  • Demonstrated case management and supervision skills.
  • Excellent communication and interpersonal skills.
  • Valid Driver’s License and proof of liability insurance.
  • Background checks and immunizations completed.

Responsibilities

  • Provide weekly supervision and guidance to RIW Case Manager staff.
  • Conduct regular staff meetings to ensure DHS compliance.
  • Provide case consultation and support to staff for complex cases.
  • Develop service plans with clients and provide community/home visitation.
  • Assist with completing funding, housing, utilities, medical and state benefits applications.
  • Maintain accurate client records per DHS standards.
  • Utilize computer systems to maintain client information.
  • Complete paperwork for client statistics.
  • Ensure care standards meet funder requirements.
  • Collaborate with other CCAP programs to coordinate services.
  • Attend program meetings and trainings.
  • Other duties as assigned.

Job Summary

The Specialized Case Manager will provide oversight and supervision to CCAP’s RIW case managers ensuring compliance with CCAP and DHS guidelines. In addition, assisting clients enrolled in Supportive Services component, who required additional coordinated range of client-centered services to ensure their compliance for specific component area and assisting with resolving barriers to progress.

WORK SCHEDULE DEMANDS

This is a full-time position 35 hours per week.

Due to the nature of the job, schedule flexibility is required to accommodate the client population.

May be required to work at any CCAP location.

KEY RESPONSIBILIITIES
  • Assist in providing weekly supervision, and guidance to CCAP’s RIW Case Manager staff.
  • Conduct regular staff meetings to ensure compliance with DHS requirements, policies, and procedures.
  • Provide case consultation and support to staff, helping them navigate complex cases and resolve challenges effectively.
  • To develop a service plan with clients per the specific program's provisions and provide community and/or home visitation to assigned clients.
  • To provide instruction in adult daily living skills as needed and as related to barriers to academic/vocational/employment goals.
  • To assist with completing emergency funding, housing, utility assistance, medical and state benefits, and other appropriate applications for assigned clients as needed.
  • Maintain current and accurate record keeping for client files following the standards of DHS.
  • To utilize computer database systems to maintain appropriate information on clients per program and agency requirements.
  • To complete all paperwork associated with maintaining current client statistics.
  • Ensure agency standards of care are met and in compliance with funders and others as required.
  • Collaborate with other CCAP programs and administration to ensure coordination of efforts and resources and the smooth implementation of services.
  • Attend program-related meetings, training, and development sessions as appropriate.
  • Other duties as assigned.

REQUIRED QUALIFICATIONS
  • BSW/BA and a minimum of 2 years related experience.
  • Must have demonstrated case management skills and experience.
  • Demonstrated supervisor skills and experience a plus.
  • Excellent communication and interpersonal skills.
  • Ability to motivate and encourage others, even in challenging situations.
  • Strong organizational skills and attention to detail.
  • Strong knowledge and use of Microsoft and Google platforms.
  • Bilingual or multilingual proficiency is a plus.
  • Provide/maintain all required immunizations and/or vaccinations
  • Complete all required background checks
  • Valid Driver’s License and proof of liability insurance
