Improves patients' quality of life by providing comprehensive clinical services within my area of expertise to ensure optimal medication use and effective resource management, while considering pharmacoeconomic principles across the healthcare system. Works in collaboration with healthcare team members to accomplish goals. Performs other duties as assigned.
Minimum Required: Bachelor of Science (B.S.) or Doctor of Pharmacy (Pharm.D.).
Preferred/Desired: Post‑graduate education in health-system pharmacy services.
Minimum Required: PGY‑1 Pharmacy Residency and/or equivalent clinical experience of three years.
Preferred/Desired: PGY‑2 Pharmacy Residency and/or Pharmacy Board Certification, BLS and ACLS certifications, and/or equivalent experience of greater than three years.
Computer literacy, interpersonal skills, general clinical awareness.
Advanced communication skills and computer skills, including Microsoft Office applications. High‑level cognitive ability to interpret and assess written medication orders to ensure optimal medication use.
Teaching skills and qualified to precept residents, students, interns, and externs.
Membership with state and/or national hospital pharmacy associations.
Current, active, unrestricted pharmacy licensure for appropriate state or eligibility for licensure. Must acquire licensure within 120 days of hire date.
Minimum Required: PHARMACIST‑FOR APPROPRIATE STATE.
