Molina Healthcare seeks a field-based coordinator to support long-term care screening processes for older adults and persons with disabilities. The role requires travel to Wisconsin member locations and monthly meetings at the Molina office.

Qualifications

  • At least 1 year health care experience serving adults with disabilities or the frail elderly.
  • LTCFS certification is required.
  • Bachelor's degree in social work/health field or RN/CNA/LPN license (where applicable).
  • Valid driver's license and reliable transportation.
  • Ability to support staff development and meet quotas.
  • Experience with data management, coordination and training.
  • Ability to explain and teach complex material and communicate clearly.
  • Proficient in MS Office and related software.

Responsibilities

  • Schedule home-based LTCFS assessments with members.
  • Use Wisconsin LTCFS to assess functional status and supports.
  • Complete LTCFS collaboratively with member, family and ICT.
  • Coordinate with ICT for enrollment and changes.
  • Ensure data consistency with member records.
  • Track LTCFS timeframes with spreadsheets.
  • Submit accurate documentation per guidelines.
  • Attend trainings to stay updated on tools and directives.
  • Participate in quality assurance and audits.
  • Maintain LTCFS certification through CST.

JOB DESCRIPTION Job Summary

The role is a hybrid role with travel to member location assigned and meeting at Molina office once a month.

The candidate must be located at one of the below Wisconsin (WI) locations:

Spooner (Washington County), Park falls (Price County), Menomonie (Dunn County), New Auburn / Eau Claire (Chippewa county), Arcadia (Trempeauleau county), Tomah (Monroe county), Reedsburg (Sauk county), La Crosse (Lax County), Viroquoa (Vernon county), Bloomington / Prairie du chein (Grant county), Madison/ Verona (Dane county).

Provides support for long-term care screening processes and coordination for older adults and adults with physical or intellectual disabilities. Contributes to overarching strategy to provide quality and cost-effective member care.

Essential Job Duties
  • Schedules appointments with members to complete home based long-term care functional screen (LTCFS) assessments.
  • Uses the Wisconsin LTCFS to gather information about member functional abilities, health status, and personal/professional supports.
  • Completes the LTCFS in a collaborative manner with the member, family, other member supports, and the member interdisciplinary care team (ICT).
  • Coordinates with member ICT to ensure new enrollment, annual, and change in member condition screens are completed.
  • Ensures consistency between data gathered during the functional assessment and member-specific data contained in the member record.
  • Uses spreadsheets to track and ensure adherence to LTCFS timeframes.
  • Submits timely, accurate, and complete documentation in accordance with established guidelines and processes.
  • Attends meetings and training as needed or required to maintain knowledge of agency processes, the LTCFS tool, and/or any changes to process or the screening tool resulting from new state directives.
  • Participates in quality assurance activities that ensure competence including regular auditing of completed work.
  • Maintains LTCFS certification by completing and passing Wisconsin continuing skills testing (CST).
  • Applies professional judgment in documentation - demonstrating respect for members and maintaining screen integrity when completing assessments.
  • Local travel may be required (based upon state/contractual requirements).
Required Qualifications
  • At least 1 year experience in health care supporting adults with physical/intellectual disabilities or the frail elderly, or equivalent combination of relevant education and experience.
  • Long-term functional screening (LTCFS) certification.
  • A bachelor’s degree in a social work, human services or health-related field, or Registered Nurse (RN) or CNA or LPN or LVN license may be required for certain states (dependent upon state/contractual requirements). If licensed, license must be active and unrestricted.
  • Valid and unrestricted driver's license, reliable transportation, and adequate auto insurance for job related travel requirements.
  • Ability to support staff professional development, meet performance quotas and balance team dynamics and independent responsibilities.
  • Experience with data management, project coordination, and training.
  • Ability to explain and teach complex and detailed material in group and individual settings.
  • Excellent verbal and written communication skills.
  • Microsoft Office suite/applicable software program(s) proficiency.
Preferred Qualifications
  • Current or former certification to conduct/administer adult long-term care functional screening (LTCFS) in the state of Wisconsin.

Pay Range: $19.84 - $38.69 / HOURLY
