Job summary

Yale New Haven Health in New Haven, CT, seeks a dedicated program coordinator to support the CT Breast, Cervical Cardiovascular and Colorectal programs. You will screen enrollment eligibility, enter data into state databases, and coordinate activities with multiple partners to meet program goals.

The role requires 3–5 years of ambulatory/community health experience, strong data and organizational skills, and the ability to engage diverse populations. Travel may be required.

Qualifications

Responsibilities

• Incorporates new initiatives and procedures to improve community health and access.
• Develops projects and programs related to chronic disease management with defined timelines.
• Collaborates with managers, community orgs, case teams for planning and evaluation.
• Consults with site personnel on administrative and clinical projects; analyzes outcomes and reports.
• Performs health training assessments, designs programs, publishes materials and grants proposals.
• Prepares, analyzes, facilitates and distributes statistical reports to leadership.
• Ensures systems and safety-net programs (Medicaid, sliding scales) are in place.
• Addresses community health issues across multicultural populations.
• Spearheads the Ecuadorian Health Program linking CT residents to reimbursed care.
• Serves on department committees to support workflow and teamwork.
• Maintains participant information spreadsheets (addresses, phone, appointments).
• Oversees day-to-day program operations and coordinates patient appointments.
• Assists with Quarterly and Final reports as required by DPH; meets performance indicators.
• Performs all other duties or special projects as assigned.

Under the direction of the Ambulatory Manager, responsible for designated Program activities which includes determining program eligibility, patient enrollment, system maintenance, interface and coordination of Program Team activities to meet the established goals. This includes pre-screening women for enrollment eligibility, working with all patients, particularly with patients with low literacy levels, entering data into the state database system. Ability to quickly process complex problems, identify key components and develop plan for resolution. Effective interactions with all healthcare team members and the ability to elicit their cooperation required.

Responsibilities

• 1.Incorporates new initiatives and procedures supported through the Department of Community Health's efforts to improve the health of the community as it relates to chronic disease management issues and improved health care access.
• 2.Develops projects and/or programs related to community based chronic disease management issues and implements and evaluates new initiatives within agreed upon established time frames.
• 3.Under the direction of the Manager, Special Projects and Clinical Support consults and collaborates with community based organizations, case management teams, and other designated staff regarding program and activity planning, implementation, and evaluation.
• 4.Consults and collaborates with personnel at collaborating sites regarding administrative and clinical systems or administrative projects or concerns. Assists in analyzing the outcomes and results and prepares reports as required.
• 5.As a health educator, performs health training assessments, design and develop successful programs, publish materials, papers and grant proposals; plan health fairs and events
• 6.Prepares, analyzes, facilitates, and distributes statistical reports to leadership and collaborating partners.
• 7.Works with appropriate entities to ensure that systems/safety net programs for under/uninsured, i.e., Medicaid Managed Care, sliding scales, etc.are in place
• 8.Ability to address community health issues especially multi cultural multi ethnic populations.
• 9.Spearhead the Ecuadorian Health Program: which ties the government of Ecuador and its citizens that reside in the state of CT with reimbursed health care services from the Yale New Haven Health System
• 10.Serve on department based committees in leadership roles to ensure fluidity of workflow, sustained camaraderie and instruction
• 11.Maintain Excel sheet of all participants information, including address, phone, appointments scheduled,etc.
• 12.Oversee day to day operations of the program, coordinate appointments, and work with team to ensure all program objectives are met.
• 13.Assist with all Quarterly and Final reports as required by DPH in a timely fashion
• 14.Meets all performance indicators set forth by the Department of Public Health
• 15.All other duties or special projects as assigned.

Bachelors degree required with Master Degree in publich health preferred

EXPERIENCE

Three (3) to five (5) years of progressively responsible experience in ambulatory services, community health, including experience in database and program coordination. 1-2 years relevant experience in community health education, outreach, patient navigation and program development helpful.

PHYSICAL DEMAND

Travel required. Must have access to transportation and knowledge of Greater New Haven community.

Additional Information

• This is a non-clinical key position which is part of the community navigation team and supports the day to day operations for the CT Breast, Cervical Cardiovascular and Colorectal programs which is a grant funded systemwide program. Supports operations with 3 subcontractors Fair Haven Community Health Center, Cornell Scott Hill Health Center and Greenwich Family Centers program. This position also supports some work in our 7 school based health centers.
