Qualifications

• Credential Required: Licensed as a Registered Nurse and an APN Nurse Practitioner.
• Meets credentialing requirements as stipulated by Medicare, Medicaid, and private insurers
• Preferred credentialed with insurance companies

Responsibilities

• Provide primary care services to adults and families.
• Work within all state and federal laws and regulations, and the agreement with the collaborative physician.
• Manage acute and chronic health problems using evidence-based protocols.
• Provide health screenings, immunizations, and other disease prevention programs.
• Provide age-appropriate assessment and interventions for patients and families.
