Job summary

PHMC in Philadelphia seeks a Family Nurse Practitioner - Bilingual (Spanish) to deliver primary care to adults and families in English and Spanish within an FQHC setting. You will report to the Medical Director and work on-site at Congreso Primary Care Center.

Requirements include RN and PA NP credentials, PA license, Spanish proficiency, and at least three years of nursing experience with APN background preferred. Competitive salary and benefits.

Qualifications

• Spanish bilingual proficiency in medical terminology.
• Strong cultural competence in Latin American populations.
• Ability to collect patient data, perform physical exams, and triage per protocols.
• Excellent verbal and written communication skills.
• Ability to work in a team and adapt to changing needs.
• Maintain documentation and reporting accuracy.

Responsibilities

• Provide primary care services.
• Work within all state and federal laws and regulations, and the agreement with the collaborative physician.
• Manage acute and chronic health problems using evidence-based protocols.
• Provide health screenings, immunizations, and other disease prevention programs.
• Provide age-appropriate assessment and interventions for patients and families.
• Provide patient/family education, health counseling, and health advocacy.
• Be willing to work occasional weekends and on-call.
• Collaborate with clinical staff on care and follow-up.
• Participate in public health management of communicable diseases in shelters and other health initiatives.
• Maintain positive relationships with funding sources and community organizations.
• Perform other related tasks and duties as directed.
• Work as a member of the Primary Care Medical Home team to provide integrated health services.
• Document all services promptly.
• Act as a mandated reporter to the State of Pennsylvania and Joint Commission
