Job summary

Washington County Behavioral Health Division, within the Department of Health and Human Services, seeks a Bilingual (Spanish) Adult Care Coordinator for adults with severe mental illness across multiple programs including residential treatment, Aid and Assist, and civil commitment.

The role coordinates care from referral to discharge, supports clients in the community and at OSH, and collaborates with social workers, health plans, and legal partners. Hybrid work arrangements may apply.

Qualifications

• Master's degree in Counseling, Psychology, Social Work, or related field.
• Three years of increasingly responsible experience in case management or services coordination.
• QMHP certification or clinical license required.

Responsibilities

• Coordinate care from referral to discharge across residential treatment, Aid and Assist, and civil commitment.
• Meet clients to assess needs and create a plan for system navigation.
• Facilitate team meetings to support transitions in care levels and discharge planning.
• Collaborate with hospital social workers, health plans/CCOs, and other partners to support clients.
