FT Intermittent LPN

As a Home Health Nurse (RN/LPN), you will be called to care when you’re needed most. Supporting a full range of patient services, you will help bring comfort and dignity to our clients.

Responsibilities of a Home Health Nurse (RN/LPN):

• Provide direct patient care according to the prescribed plan of care and scope of practice, monitoring progress and recovery.
• Assess and document patient conditions, reporting changes as needed (vitals, blood/stool/urine, glucose, etc.).
• Administer medication, insulin, IV fluids, and document thoroughly.
• Inspect and care for wounds, change dressings, and assist with personal hygiene.
• Assess for injuries, infections, bedsores, etc., and deliver appropriate treatments.
• Collaborate with healthcare providers and staff to ensure optimal patient outcomes.

Qualifications for Home Health Nurse (RN/LPN):

• Associate degree in Nursing (ADN) or AAS required.
• Current unrestricted RN or LPN license in the state of employment.
• Current CPR/AED/BLS/First Aid certification.
• Reliable transportation to care sites.
• Practical trach and/or ventilator experience preferred but not required.
