Skin/Wound Ostomy Clinician

About the Job:

As a member of the Patient Care Services team, the Skin/Wound Ostomy Clinician assesses and evaluates the patient’s skin status. He/She develops age-specific and culturally appropriate plans of care, interventions and evaluations based on assessment of the patient’s needs. The Skin/Wound/Ostomy Clinician identifies and utilizes age specific teaching techniques appropriate to patient’s and family’s developmental level. He/She facilitates professional development of clinical staff. He/She demonstrates emerging leadership behaviors through involvement in area activities and performance improvement efforts. The Skin/Wound/Ostomy Clinician works in collaboration with the healthcare team and is accountable to the Director of the Wound Ostomy Program.

What You’ll Do:

  • Responds to referrals to identify patient’s skin, wound, and ostomy needs
  • Conducts advanced assessments of patient’s skin, acute and chronic wounds, stomas and fistulas
  • Identify modifiable and non-modifiable risk factors so preventative measures can be appropriately implemented.

Who You Are:

  • Graduate from an accredited school of nursing. Completion of BSN or higher nursing degree by 2025
  • Current NH Licensure as a registered nurse.
  • Minimum of three years as a Wound Ostomy Clinician or specialty certification in wound or wound/ostomy care.
  • Must possess good communication skills to work with clients, community services, and the health care team.
  • Ability to achieve the following competencies: Basic Life Support, Care of the complex ostomate, Management of the complex wound, Vacuum Assisted Closure (VAC) Therapy, Compression Therapy, Mentoring, Must complete a minimum of six hours of continuing education in the management of the skin, wounds and ostomies.
