Job summary

A community mental health organization in New York is seeking a Peer Specialist to provide outreach support for individuals with mental health and housing needs. Qualifications

• Experience working with homeless and/or precariously housed populations preferred but not required.
• Knowledge of homeless resources, NYC shelter systems, and MTA transit systems is a plus.
• Ability to prepare accurate and timely reports.

Responsibilities

• Conduct persistent and assertive outreach and engagement.
• Assess health and social needs of participants.
• Participate in hospital discharge planning meetings.
• Monitor and evaluate participants' progress.
