Overview

The Peer Support Specialist provides outreach, engagement, and recovery support services to individuals eligible for the SOR program. Using lived experience and a recovery‑oriented approach, this position offers one‑on‑one and group peer support, education, advocacy, and guidance to individuals and their families. The Peer Support Specialist collaborates with the multidisciplinary treatment team to coordinate care, assess ongoing needs and risks, connect individuals to community resources and self‑help supports, and promote sustained recovery and treatment engagement. Responsibilities include making appropriate referrals, supporting recovery goal development, documenting services provided, and helping individuals access resources that address substance use, mental health, family, financial, and other social determinants impacting overall well‑being.

Key Responsibilities

• Works as part of the SOR program, to provide outreach to individuals who may be eligible for SOR and to provide peer services to program participants.
• Provides practical advice, suggestions, and strategies of recovery to patients as an empowering helper with the perspective of being a recipient of services in the past.
• Provides one‑on‑one support and group sessions.
• Works with individuals and families to prevent crisis and promote treatment engagement while assessing risk ongoing. May help individuals deal with addictions and substance use; family, parenting, and marital problems; suicide; stress management; financial issues; and issues associated with aging and mental and emotional health.
• Must be very familiar with and actively refer to various social services resources in Charlotte and surrounding counties.
• Provides hope, support, education, and assists patients in setting recovery goals.
• Provides input, as part of the treatment team, on the patient’s care.
• Work closely with SOR CM and other members of the multidisciplinary treatment team to coordinate services for individuals determined to be in need.
• Makes appropriate internal and external referrals, monitors the implementation of services, and provides support and advocacy for the individual.
• Assists with linking patients to self‑help groups and community supports that help sustain continued recovery.
• Documents services in the patient record.

Qualifications

• High School Diploma or equivalent.

• Peer Specialist certification eligible or obtainable within one (1) year of hire date.
• Ability to manage stressful situations and display appropriate work demeanor and boundaries.
• Demonstrates excellent customer service.
• Maintains high standards of ethical and professional conduct, while adhering to agency policies and procedures.
• Possesses a valid Florida driver’s license reflecting a safe driving record and personal vehicle use.

• Strong oral and written communication skills.
• Ability to work both independently and as part of a team.
• Can lift at least 10 pounds.
• Moderate manual dexterity.
• Can move quickly during a crisis.
