Summary of Job Duties: The Clinical Transplant Coordinator is a professional registered nurse and a designated member of the transplant team whose primary responsibilities are directly involved with coordinating the clinical aspects of care. The coordinator works with the patients and their families through and after transplantation, in a compassionate and tactful manner in order to facilitate access to and provide continuity of care. The coordinator also works with the other member of the transplant team including physicians, nurses, social workers, dietitians, financial coordinators and administrative personnel. The RN will be responsible for organizing and managing care of transplant patients outside of hospital and acting as liaison for Inpatient discharge plan of care. Participates in quality and performance improvement processes. Assists in providing education to patients, families and staff regarding transplant and associated plan of care.
Qualifications: Minimum Qualifications:
  • Graduate of an Accredited School of Nursing.

  • Four (4) years of nursing experience.

  • Current Arkansas State Board of Nursing License.

Preferred Qualifications:

  • Care Coordination and Transition management, or Outreach/multidisciplinary clinical education


Additional Information:

Responsibilities:

  • Collaborates and communicates with various members of the transplant team to coordinate transplant process.

  • Facilitates acceptance or refusal of donor organs for transplant and coordinates the organ recovery.

  • Prepares patients for organ transplants by assessing implications, participating in education sessions, performing physical assessments, and coordinating care.

  • Participates in quality assessment and process improvement activities.

  • Provides input to leadership in the standardization of training and procedures related to call support and after-hours organ allocation processes.

  • Participates in community outreach and acts as a resource person regarding transplantation.

  • Performs other duties as assigned.
