Job summary

Qualifications

• Bachelor of Science in Pharmacy or Doctor of Pharmacy degree.
• Completion of an ASHP accredited post-graduate year 1 residency program or 3 years of related experience.
• Licensed to practice pharmacy in Texas or eligible; must be licensed within 90 days of hire.

Responsibilities

• Assist with transplant pharmacy referrals and transitions of care.
• Educate patients on medications and adherence.
• Assess regimens, optimize outcomes, and ensure cost-effective therapy.
• Collaborate with physicians and nurses to support coordinated, patient-centered care.
• Participate in patient rounds, quality projects, and clinical pathway development.
