Job Summary

As a member of a multi-disciplinary team, you will be responsible for assessing and providing support and intervention for the social and emotional needs of patients/residents and their families to promote continuity of care and services. Work includes conducting psychosocial assessments and interventions, mental health screens and referrals and discharge planning for Acute Care patients, Swing Bed residents and patients in the Emergency Department.

Job Duties and Responsibilities
  • Work collaboratively with providers, nursing staff and CNAs, therapy department, pharmacists, clinic case managers, community resources and agencies.
  • Communicate patient needs to assist the treatment team in advancing patient care and discharge goals. Develop discharge plan through collaboration with interdisciplinary team.
  • Meet face to face with patients/families/patient representatives to assist with discharge planning and cross‑continuum of care.
  • Conduct psychosocial assessments and mental health screens in the Emergency Department and assist with appropriate referrals of care, i.e., contacting community providers to establish continuity of care, referrals to the Designated Crisis Response Team and/or facilitate inpatient psychiatric admissions.
  • Collaborate with internal and external resources to facilitate a safe transition from hospital to another facility or home with appropriate services. Identify discharge barriers and initiate action to resolve them.
  • Maintain timely documentation in patient/resident electronic record.
  • Perform special projects and other related duties as assigned.

Required Education and Experience
  • Master’s degree in Social Work from an accredited school of Social Work required.
  • Two (2) years of recent hospital or clinical experience preferred.
Required Licenses, Certifications and Registrations
  • Licensed as a LICSW (Licensed Independent Clinical Social Worker) in the State of Washington is preferred, will substitute license for 3 years of relevant hospital work experience.
  • Current Basic Life Support (BLS) certification required.

  • Advanced knowledge of social work and behavioral health theories and practices. Demonstrates competence in clinical skills such that the individual can perform required tasks.
  • Ability to assess patient needs and exercise independent judgment in managing and coordinating care.
  • Ability to cultivate effective partnerships and work in collaboration with providers, other health care professionals, caregivers, and patients. Strong leadership, facilitation, delegation, and coaching skills.
  • Proficient in the use of current technology, including Microsoft Office products. Ability to learn and effectively use electronic medical records and other systems and equipment.
  • Professional and effective written and verbal communication skills. Ability to identify and employ communication strategies appropriate to the audience. Ability to maintain accurate documentation and records.
  • Ability to work independently performing a wide variety of assignments that require the use of independent judgment, problem‑solving, organization, and prioritization skills.
  • Strong leadership, organizational and analytical skills.
  • Ability to manage multiple priorities and display flexibility in a fast‑paced and changing work environment.
