Job summary

Schenectady Center is hiring a Social Worker in Schenectady, NY. You will support residents’ social and emotional needs, help develop individualized care plans, and participate in admissions, re-entry, or transfers, working within an interdisciplinary team to ensure holistic resident care.

• Participating in the planning of the resident’s admission, community re-entry or transfer to another facility
• Participating in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident

REQUIREMENTS:
