Job summary

The Riverside Premier Rehabilitation and Healing Center in New York, NY seeks a Social Worker to join a busy interdisciplinary team in a rehabilitative and long-term care setting. You will assess psychosocial needs, facilitate care planning, and support residents and families through discharge transitions.

The role requires collaboration with clinical and administrative staff and a commitment to resident rights and quality of life.

Qualifications

• Excellent communication and interpersonal skills.
• Experience in discharge planning and care coordination.
• Knowledge of PCC and MDS is a plus.
• Ability to manage multiple priorities in a fast-paced setting.

Responsibilities

• Assess residents’ social, emotional, and psychosocial needs.
• Develop and implement individualized care plans.
• Participate in interdisciplinary care plan meetings.
• Coordinate admissions, transfers, and discharge planning.
• Assist with securing home care services, DME, and community resources.
• Provide support and guidance to residents and families throughout their stay.
• Collaborate with clinical and administrative teams to ensure continuity of care.
• Assist with financial documentation and resident education as needed.
• Advocate for resident rights and quality of life.
