Carease Health is seeking a part-time Social Worker to provide emotional support and brief therapeutic interventions to residents in skilled nursing facilities in Massachusetts. You will visit designated facilities on a set schedule, perform psychosocial assessments, and deliver short-term therapy while maintaining detailed records.

Qualifications

• Associates, Bachelor's or Master's degree in Social Work, Psychology, Counseling, or another mental health-related field.
• Understanding of geriatric mental health, crisis management, and psychosocial support in long-term care settings.
• Strong communication, documentation, and relationship-building skills.
• Education: Associate Degree
• Experience: 1 year

Responsibilities

• Conduct assessments of residents to identify emotional, cognitive, and psychosocial needs
• Implement short-term, solution-focused therapy and supportive interventions to address immediate concerns
• Document visit notes and maintain detailed records of interactions and interventions after each visit
• Collaborate with psychiatric professionals to ensure comprehensive care, including participation in a monthly meeting with the psychiatric NP

About the Role

Our experienced clinical team operates seamlessly with your facility's processes and protocols to ensure minimal disruption to your current operations.
