We are seeking a compassionate and dedicated Mental Health Provider to join our Collaborative Care team in Greenville, PA.
As a Care Companion/Social Worker, you will be responsible for visiting designated facilities, conducting assessments, and implementing short-term, solution-focused therapy and supportive interventions to address the emotional, cognitive, and psychosocial needs of our clients. You will also document visit notes, maintain detailed records, and collaborate with psychiatric professionals to ensure comprehensive care.
Visit designated facilities in accordance with the schedule set with your supervisor
Conduct assessments of residents to identify emotional, cognitive, and psychosocial needs
Implement short-term, solution-focused therapy and supportive interventions to address immediate concerns
Document visit notes and maintain detailed records of interactions and interventions after each visit
Collaborate with psychiatric professionals to ensure comprehensive care, including participation in a monthly meeting with the psychiatric NP
Associates, Bachelor's or Master's degree in Social Work, Psychology, Counseling, or another mental health-related field
Understanding of geriatric mental health, crisis management, and psychosocial support in long-term care settings
Strong communication, documentation, and relationship-building skills
Education: Associate Degree
Experience: 1 year

Regular supervision and collaboration with psychiatric providers
