Responsibilities
Participate in an interdisciplinary team approach.
Provide therapeutic intervention for residents.
Maintain written documentation in the resident medical record.
Perform an initial evaluation of each resident's psychosocial needs.
Develop social service component of the resident's care plan.
Coordinate interdisciplinary care planning conferences.
Act as a liaison between residents, families, and agencies.
Facilitate discharge planning.
Concern self with the safety of all Center customers.
Coordinate admissions to the facility.
Develop and maintain relationships with referral sources.
Verify insurance coverage.
Conduct tours of the facility.
