Job summary

Qualifications

Responsibilities

• Participate in an interdisciplinary team approach.
• Provide therapeutic intervention for residents.
• Maintain written documentation in the resident medical record.
• Perform an initial evaluation of each resident's psychosocial needs.
• Develop social service component of the resident's care plan.
• Coordinate interdisciplinary care planning conferences.
• Act as a liaison between residents, families, and agencies.
• Facilitate discharge planning.
• Concern self with the safety of all Center customers.
• Coordinate admissions to the facility.
• Develop and maintain relationships with referral sources.
• Verify insurance coverage.
• Conduct tours of the facility.
