In this role, you will provide essential psychosocial support to residents and their families, ensuring the highest quality of life for our residents through personalized care and counseling.

Pay Range: $66,000.00 - $70,000.00 per year

Key Responsibilities

Resident & Family Support

• Conduct interviews with residents and their families to assess psychosocial needs.
• Provide individualized psychosocial assessments, counseling, and guidance to residents and their families.
• Offer grief and loss counseling to residents and families experiencing difficult transitions.

Care Plan Development

• Develop personalized care plans tailored to the individual needs of residents, in collaboration with the healthcare team.
• Assist residents in navigating and applying for appropriate social services and community resources.

Crisis Intervention & Support

• Provide crisis intervention services as needed to address immediate concerns or challenges.
• Offer supportive counseling for emotional and psychological well-being.

Communication & Collaboration

• Strengthen communication between residents, families, and facility staff to ensure a coordinated approach to care.
• Collaborate with other healthcare professionals to ensure holistic care for each resident.

Required Qualifications

• Bachelor’s Degree in Social Work or a related field (Master’s preferred).
• Previous experience in a healthcare or rehabilitation setting is required.
• Strong interpersonal and communication skills, with the ability to build rapport with residents, families, and colleagues.
