Job summary

Cheshire West and Chester Council is recruiting for a 18.5-hour Social Worker in the Home First Intermediate Care Team. The post supports independence after hospital discharge within a proactive, multidisciplinary setting. Grade 9 applications are invited, with possible consideration for newly qualified Grade 8s.

Qualifications

• Professional Social Work qualification.
• Experience of managing high risk, complex cases, and safeguarding Adults.
• Ability to communicate effectively with customers, carers and families.
• Understanding of services essential to meeting the needs of people with Mental Health needs.

Responsibilities

• Work in a fast-paced intermediate care team integrated with health colleagues.
• Promote independence following hospital discharge and support planning.
• Collaborate with multidisciplinary teams to identify options for care.
• Ensure people are connected to support services and community assets.
