Job summary

Parsons Hill Rehabilitation & Health Care Center in Worcester, MA is seeking a full-time Social Worker to join our care team. The role focuses on planning, directing social services to meet residents’ emotional and social needs and coordinating with families during placement and crisis.

Requirements include a MA social work license (LSW/LICSW/LSWA/LCSW) and a bachelor’s in social work; ideally 1 year of hospital/health-related social work.

Qualifications

• Must hold a MA social work license (LSW, LICSW, LSWA or LCSW).
• Bachelor’s degree in social work from an approved school.
• Prefer minimum 1 year of hospital or health-related social work experience.

Responsibilities

• Coordinate and monitor resident care conferences.
• Advise residents, families, and responsible parties regarding placement and crisis adjustment.
• Protect residents' health information and ensure timely documentation.
• Advocate for residents’ personal needs and coordinate community resources.
• Support staff with resident-related issues.
