$23.70/hr - $46.35/hr

Responsibilities
  • Conduct telephonic outreach to meet quality and production metrics

  • Collaborate with team members, members, PCPs, and specialists
  • Assist with provider outreach, care gap notifications, and medical record requests
Qualifications
  • Bachelor’s in Social Work (BSW) from an accredited program
  • Active, unrestricted LBSW license in the work state
  • At least 2 years of active social work experience
  • Healthcare clinical background of 2+ years
  • Proficiency in Microsoft Office Suite
Preferred Qualifications
  • Master’s degree in Social Work from an accredited program
  • Unrestricted LMSW license
  • Experience with geriatric populations
  • Bilingual skills, especially in Spanish or Vietnamese
