Job summary

Havenwyck Hospital in Auburn Hills, Michigan, seeks a licensed Social Worker to join our inpatient team on a PRN basis. The Social Worker provides social services to all inpatient and day hospital patients, facilitating care coordination and discharge planning.

The role supports a multidisciplinary treatment approach under the supervision of a psychiatrist. Michigan LMSW/LLMSW or equivalent license is required, with inpatient psychiatric experience preferred and a strong commitment to

Qualifications

• MSW degree from a CSWE-accredited school, and/or an MA degree, is required.
• Current License in Michigan as LMSW/LLMSW, LPC, or LLC is required.
• Experience on an inpatient psychiatric unit preferred.
• Employee must be in good standing to be considered for internal transfers.

Responsibilities

• Participates as an integral member of a multidisciplinary treatment team led by the psychiatrist.
• Facilitates the therapeutic milieu on the treatment unit.
• Performs social work interventions as directed by the team and treating psychiatrist.
• Obtains and assesses social history information and reports to the treatment teams.
• Performs discharge planning/case management from admission.
• Provides group/individual/family therapy or related modalities as deemed appropriate.
• Acts as liaison for hospitalized patients with community agencies and resources.
• Attends treatment team conferences and completes treatment plans.
• Documents in the medical record.

Position Summary

Havenwyck Hospital is seeking a dynamic and talented licensed Social Worker to join our team of compassionate and dedicated clinical professionals. The Social Worker provides social services to all inpatient and day hospital patients.

Social Work Duties Include, But Are Not Limited To

• Participates as an integral professional member of a multidisciplinary treatment team led by the psychiatrist.
• Facilitates the therapeutic milieu on the treatment unit.
• Performs social work interventions as directed by the treatment team, the treating psychiatrist and/or the Lead Social Worker.
• Obtains and assesses social history information and reports this to the treatment teams.
• Performs discharge planning/case management from the day of admission.
• Provides group therapy, individual therapy, family therapy, substance abuse group/didactic, and/or couples therapy as deemed appropriate by the treatment team, the treating psychiatrist, Director of Clinical Services and/or Lead Social Work.
• Acts as liaison for hospitalized patients with various community agencies and resources.
• Attends treatment team conferences and completes treatment plans.
• Documents in the medical record.

Qualifications

• Master of Social Work degree from a school of Social Work accredited by the Council of Social Work Education, and/or a MA degree required.
