Job summary

Havenwyck Hospital (a UHS facility) in Auburn Hills, MI, is seeking an Outpatient Clinician (Clinical Social Worker) to join our behavioral health team. You will work with a multidisciplinary treatment team to perform patient assessments, develop treatment plans, provide individual, family and group therapy, discharge planning, and care coordination.

Requires a MSW or MA in Counseling and Michigan LMSW/LPC license; full-time schedule Mon-Thurs 12n-8pm and Fri 8am-4pm; benefits include PTO,

Qualifications

• Master of Social Work degree required.
• Master of Arts in Counseling degree required.
• Michigan LMSW or LPC license required.

Responsibilities

• Be part of a multidisciplinary treatment team and provide social services.
• Perform patient assessments, treatment planning, individual/family/group therapy, and discharge planning.
• Coordinate supports and collateral contacts as needed.
• Attend weekly team and milieu meetings; participate in Performance Improvement activities.

Responsibilities

The Outpatient Clinician (Clinical Social Worker) provides social services to patients under general direction of the Director of Outpatient Services. The Clinical Social Worker is a member of the multidisciplinary treatment teams. Duties may vary somewhat depending on experience and areas of specialization, however, the listed duties and responsibilities are characteristic of the position.

Duties Will Include

• Participates as an integral professional member of a multidisciplinary treatment team.
• Effectively and professionally engages in the provision of and clinically documents patient assessment, treatment planning, individual therapy, family therapy, group therapy, and discharge planning.
• Engages in supports coordination and collateral contact as indicated by patient needs during programming.
• Works a schedule that meets the needs of the program.
• Attend weekly treatment team meetings.
• Attends program milieu meetings.
• Participates in Performance Improvement activities.

Qualifications

• Master of Social Work degree from a school of Social Work or Master of Arts in Counseling degree required.
• Current License in the State of Michigan as a Fully Licensed Social Worker (LMSW) or LPC required.
• Employee must be in good standing (employment/corrective action history, performance evaluation, etc.) to be considered for an internal transfer.
