Job summary

Oak Crest Village by Erickson Senior Living in Parkville, MD is seeking a Social Worker to provide social work services to residents and families in Continuing Care. This role supports admissions, discharge planning, and transitions while educating on advance directives.

You'll deliver psychosocial support, participate in care/service plan meetings, and offer bereavement support. Schedule is PRN/Flex with compensation at $40/hour. Equal opportunity employer.

Qualifications

• Social Work degree (BSW or MSW) or state-licensure equivalent.
• Minimum 3 years’ experience with older adults in post-acute/long-term care.
• Knowledge of geriatric population and dementia.

Responsibilities

• Complete admission assessments, MDS, and required documentation per state and federal regulations.
• Educate residents and families on advance directives and facilitate completion, signature, and tracking of documents.
• Coordinate the Social Work function from admission to discharge including room changes and transition planning.
• Provide psychosocial support to residents and families to ensure a successful transition to Continuing Care.
• Participate in care/service plan meetings with family members.
• Provide bereavement support to residents, family members, and staff.

Location:

Oak Crest Village by Erickson Senior Living

Join our team as a Social Worker. In this role, you will be responsible for providing social work services to the residents and family members of Continuing Care.

Schedule:

PRN/Flex arrangement to support the team with covering vacation, sick time, or medical leave absences from full-time team members during typical office hours.

Compensation:

$40.00 hourly.

How you will make an impact

What you will need
