Job summary

Intermountain Health in Lehi, Utah, seeks a Clinical Mental Health Professional to perform psychosocial assessments, develop and implement plans of care, and coordinate crisis interventions for patients and families. This role collaborates with the care team and outside agencies to ensure timely discharge and continuity of care.

The position involves crisis and inpatient therapy support, with a $500 sign-on bonus for external applicants with 1+ year of experience.

Qualifications

  • Master’s degree in social work or related field from an accredited institution.
  • Current state licensure as CSW, LCSW, or LMFT.
  • Excellent written and verbal communication skills.
  • Computer skills including email and word processing.

Responsibilities

  • Diagnoses and treats various mental, emotional, and behavioral disorders, modifying care plan to reflect changes in patient status.
  • Refers patients to other services to improve their well-being.
  • Work with the multi-disciplinary team to develop a discharge plan for continuity of care.
  • Performs behavioral health consultation for patients and caregivers; conducts psychosocial assessments; provides crisis, bereavement, and other interventions as needed.
  • Educates, communicates, and coordinates with multiple patient care teams and outside agencies to ensure timely discharge and quality care.
  • Credentialing through medical staff office and peer review may apply if in behavioral health setting.
  • May provide staff support and debriefing with Employee Assistance Program after adverse events.

Job Description:The Clinical Mental Health Professional utilizes clinical expertise to perform psychosocial assessments, develop and implement plans of care appropriate care team, assess crisis situations to appropriately provide needed resources and or referrals for the patient, and to provide family and individual treatment as indicated. This position works collaboratively with patients, patient's significant other, families, healthcare providers, insurers, community resources, and all other involved parties. Credentialing through medical staff office and peer review requirements may apply if working in the behavioral health setting.May provide support and debriefing, in conjunction with the Employee Assistance Program, for staff members following adverse events.Minimum QualificationsMaster’s degree in social work or related field. Education must be obtained from an accredited institution. Degree will be verified.Current state licensure (as required by state) as CSW, LCSW, LMFT.Excellent written and verbal communication skills.Computer Skills, i.e., email, word processing.Preferred QualificationsExperience with patients, families, and treatment teams.Dual licensure with current state licensure as LACMontana Only: Current state licensure as a MHPP.Physical RequirementsOngoing need for employee to see and read information, labels, assess patient needs, operate monitors, identify equipment and supplies.Frequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations.Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use and typing for documenting patient care, accessing needed information, medication preparation, etc.Expected to lift and utilize full range of movement to transfer patients. Will also bend to retrieve, lift, and carry supplies and equipment. Typically includes items of varying weights, up to and including heavy items.Need to walk and assist with transporting/ambulating patients and obtaining and distributing supplies and equipment. This includes pushing/pulling gurneys and portable equipment, including heavy items.
