Intermountain Healthcare seeks a Clinical Social Worker in Ogden, UT to assess psychosocial needs, diagnose and treat mental health conditions, and coordinate comprehensive care plans with the multidisciplinary team.

The role requires a doctorate in psychology or equivalent licensure (LCSW/LMFT/CMHC) and will involve crisis assessment, referrals, and documentation. A $2,500 sign-on bonus is offered to external candidates; credentialing occurs prior to start.

Qualifications

• Doctorate degree in Psychology from an accredited institution.
• Current license as a psychologist in state(s) where care is delivered.
• LCSW, LMFT or CMHC licensure as required by state regulations.

Job Description:The Clinical Social Worker utilizes clinical expertise to perform psychosocial assessments, develop and implement plans of care appropriate care team, assess crisis situations to appropriately provide needed resources and or referrals for the patient, and to provide family and individual treatment as indicated. This position works collaboratively with patients, patient's significant other, families, healthcare providers, insurers, community resources, and all other involved parties.To show our commitment to you, we are pleased to offer a $2500 sign on bonus (external candidates only)Selected candidate will need to complete internal credentialing prior to starting, current timeframe is 60-90 days.Essential FunctionsAssesses the patient's medical history, mental health history, and current presentation to understand the patient's needs from treatment.Diagnoses and treats various mental, emotional and behavioral disorders, which may include psychotherapy or psychological testing.Develops a plan of care either individually or with input from the patient, patient family/significant others, physicians and other members of the healthcare team.Provides treatment based on patient need and evidenced-based interventions.Refers patients to other services to improve their well-being. This includes frequent computer use and typing for documenting patient care, accessing needed information, medication preparation, etc.Need to walk and assist with transporting/ambulating patients and obtaining and distributing supplies and equipment. This includes pushing/pulling gurneys and portable equipment, including heavy items.
