Job summary

CommonSpirit Health is seeking a Social Worker to help patients and families navigate healthcare needs with compassion. You will assess psychosocial and discharge planning needs and coordinate safe transitions with the care team.

You will advocate for patients, connect them with community resources, and educate families on available services to support ongoing well-being beyond the hospital.

Qualifications

• Certified Social Worker (CSW) in Nebraska.
• MSW or BSW with intent to obtain a Master's.
• BLS from the American Heart Association.

Responsibilities

• Assess patients' psychosocial, environmental, financial, and discharge planning needs to develop individualized plans of care.
• Collaborate with the interdisciplinary care team to coordinate safe, effective discharge plans and referrals.
• Advocate for patients and families by connecting them with community and financial resources.
• Educate patients, families, and healthcare team on services and care options for successful transitions.
• Document assessments and care coordination while complying with standards.
