Then look no further! Glacier Hills is the place for you. We are seeking a Part Time Social Worker to join our Social Services Department.

About The Role

Your responsibilities would include but are not limited to:

• Participate in discharge planning, development, and implementation of social care plans and resident assessments.
• Perform cognitive evaluations and assess residents' moods, ensuring proper follow-up care or treatments are offered.
• Perform administrative duties such as completing medical forms, reports, evaluations, studies, charting in a timely manner, and submitting to the Director.
• Refer residents and families to appropriate social service agencies, provide information on Medicaid, Medicare, and other financial assistance programs available to residents.

About You

You must have/be:

• A Licensed Social Worker in Michigan.
• A bachelor's or master's degree in social work or a related human services field (e.g., sociology, rehabilitation counseling, psychology).
• One year of supervised social work experience in a healthcare setting preferred.
• Previous experience working in long-term care or with the geriatric population preferred.
