Job summary

Legacy Hospice is seeking a Master of Social Work (MSW) to provide patient- and family-centered social work services in a hospice setting. The MSW will deliver counseling, guidance, and support to enhance emotional well-being during challenging times.

The position is Part Time - 24 Hours per Week with coverage across Frederick, Clarke, Warren and Shenandoah counties. Strong teamwork with RN Case Manager and Bereavement Coordinator is essential.

Qualifications

• Must hold a Master of Social Work (MSW) from an accredited program.
• Licensed Social Worker preferred; active license strongly encouraged.
• Valid driver's license, vehicle insurance, and access to a dependable vehicle.
• Experience in hospice or healthcare settings preferred.

Responsibilities

• Be a supportive guide: assess psychosocial status and communicate with the RN Case Manager and interdisciplinary team.
• Connect with patients and families to evaluate medical needs, home situation, and resources.
• Develop and implement individualized social work plans in collaboration with the Bereavement Coordinator.
• Attend interdisciplinary meetings and contribute to care planning to address psychosocial stress.
• Provide empathetic counseling to patients and families.

Responsibilities

Qualifications

Education

• Required: Master of Social Work from an accredited school of social work

License Requirements

• Preferred: Licensed as a Social Worker from the State Board of Social Work
• Required: Current Driver's License, vehicle insurance, and access to a dependable vehicle

Experience

• Preferred: Minimum one (1) year experience as a social worker in a hospice setting
• Required: Minimum one (1) year experience as a social worker in a healthcare setting; long term care or medical surgical/acute care setting preferred

$31.00 - $33.00 per hour (Average Pay Range). The pay range listed represents a general guideline for the role and is not a guarantee of the final offer. Compensation will be determined based on the selected candidate’s relevant experience and the specific responsibilities of the position. Final compensation rate will be discussed and confirmed at the conclusion of the interview process.
