CentraCare Health is seeking a dedicated Social Worker to support Observation and Surgical patients on-site. You will collaborate with an interdisciplinary team to plan care, monitor length of stay, and connect families with community resources.

This role offers a stable full-time schedule and a competitive hourly pay structure with bonuses. Qualifications include Minnesota LSW licensure, a social work degree, and 2+ years of clinical social work experience.

Qualifications

• Minnesota LSW license required.
• Bachelor's or Master's degree in Social Work required.
• 2+ years of clinical Social Worker experience.
• Valid driver's license and driving record in good standing.
• ACMA Case Manager-SW Certification required within five years.

Join the care management team at St. Cloud Hospital and be a social worker to support our Observation and Surgical patients.

Our Socials Workers serve patients and families in our community and beyond by helping them during a challenging time in their lives. Our Social Workers enjoy the variety of cases they are presented with as there is always something new and different. You'll collaborate with other Social Workers to gain knowledge and experience and work with an interdisciplinary teamof nurses, therapists, physicians, and dieticians. You will have a designated patient caseload and will plan care to effectively meet the patient/family transitional needs including monitoring the length of stay and promoting community resources.

Schedule

• Full-time | 80 hours every two weeks | Onsite
• Day shift | Monday through Friday | 7:30am-4:00pm

Pay & Benefits

• Pay based on experience and degree
  • Bachelor’s in Social Work starting at $32.09 per hour
  • Master’s in Social Work starting at $33.15 per hour
  • Pay range: $30.65 - $49.73 per hour
• $3,000 hiring bonus and $5,000 relocation bonus for eligible candidates!

Qualifications

• Current Licensed Social Worker (LSW) in Minnesota required.
• Bachelor's or Master's degree in Social Work required.
• 2 years or more of clinical experience as a Social Worker.
• A valid driver's licensed and a driving record in good standing required.
• American Case Management Association (ACMA) Case Manager-SW Certification required within five years.

To be eligible for the hiring and relocation bonus you must be an external candidate who is hired into an eligible full or part time benefited position. Previous employees qualify if they have not worked for CentraCare for at least 9 months at the date of hire.
