Reporting to the Chief Care Services Officer, ALS United Greater New York is seeking an experienced licensed social worker to provide comprehensive support services and coordinate multi-disciplinary care to improve the quality of life for patients and their families living with ALS in Northern and Central New Jersey. ALS is a fatal neurodegenerative disease that requires a compassionate and committed social worker comfortable with end-of-life care.
This full-time, home-based position involves conducting virtual and in-person home visits, offering social work assessments, short-term supportive counseling, case management, and crisis intervention. Weekly in-person clinic roles in New Jersey are also required.
The position includes co-facilitating support groups for patients and caregivers, coordinating monthly educational programs (currently virtual), and participating in chapter-wide events, some on weekends and evenings.
