Job summary

Jobtailor is seeking an LMSW-licensed behavioral health clinician in New York to assess, treat, and coordinate care within an integrated team. You will use CBT, Motivational Interviewing, IPT, and behavioral activation to support patients referred by PCPs. Strong documentation and collaboration with health homes are essential.

Spanish bilingual skills are strongly preferred, and you will participate in supervision and team case conferences to ensure high-quality care.

Qualifications

• LMSW Licensure in NY or eligible State licensure.
• Master of Social Work degree from an accredited program.
• Proficient with behavioral health screening tools and biopsychosocial assessments.
• Able to apply DSM-V criteria to diagnose and treat behavioral health conditions.
• Assess risk and collaborate with psychiatric and clinical teams for risk management.
• Experience documenting clinical services in electronic health records.
• Clear, concise communication with integrated care teams including PCP, NP/MD, and Health Home staff.
• Bi-lingual Spanish strongly preferred.
