Position Overview:

Social Workers in various departments and clinics respond to referrals from the interdisciplinary team. They complete psychosocial assessments, develop and implement care plans addressing patients' needs, and facilitate transitions to the next level of care. They also provide education, consultation, and act as liaisons regarding the emotional, psychological, social, financial, and developmental aspects of patient and family care.

Minimum Qualifications:

  • Master of Social Work (MSW) from an accredited School of Social Work.
  • Licensed by the State of Ohio Social Work Board (LSW, LISW, or LISW-S).
  • For UCMC Psychiatric Services employees, NCI-APS Certification must be obtained within 90 days of hire.
  • Minimum of 1-2 years of relevant experience; preferred 3-5 years.

Responsibilities:

Social Workers will:

  • Engage patients and families into a helping process, assessing psychosocial and mental health needs.
  • Identify barriers to treatment, enhance health status, and increase satisfaction.
  • Develop rapport for ongoing support.
  • Adhere to departmental tasks and responsibilities.

Develop and implement intervention plans, including:

  • Providing counseling and social work interventions related to crisis management, mental illness, adjustment, material assistance, healthcare decision-making, and other psychosocial barriers.
  • Advocating, mediating, and negotiating to maintain or improve health status and social supports.
  • Providing family education and assistance with Advance Care Planning.
  • Referring to community resources and facilitating discharge processes.

Additional responsibilities include:

  • Summarizing care plans and addressing patient/family questions.
  • Linking patients to community resources.
  • Participating in quality improvement and evaluating patient outcomes.
  • Assessing transportation needs and arranging as necessary.
  • Referring to disease-specific agencies for employment, financial, or support services.

Further duties involve:

  • Providing education and consultation to healthcare professionals on psychosocial issues.
  • Collaborating with team members for departmental development.

Competencies:

  • Participate in activities to improve customer service and satisfaction scores.
  • Attend meetings, training, and continuing education.
  • Maintain accurate documentation and reports.
  • Respond to emails and participate in safety initiatives.
