Job summary

Amedisys Home Health is seeking a Social Worker MSW - Licensed for Hospice, full time 1st shift. You will support patients and families with social work services, care planning, and coordination across the care team to help patients heal at home.

Responsibilities include addressing financial, social, and emotional needs, educating families on interventions, and ensuring timely communication of clinical changes. Requires MSW and CPR certification, with state/federal eligibility.

Qualifications

• Must hold a Master’s degree in Social Work (MSW).
• Minimum of one year of social work experience in a healthcare setting.
• Current CPR certification.
• Meets state and federal requirements for this position.

Responsibilities

• Assists patients and families with health-related financial, social, and emotional concerns and supports coping strategies.
• Provides social work services per the plan of care.
• Ensures changes in the patient’s clinical status are communicated to the healthcare team and care center staff in a timely manner.
• Educates patients and family members and other team members on social work interventions and treatment plans.
• Performs care planning, transfer and discharge functions.
• Identifies risks to the patient’s health and communicates them to the physician and team to coordinate care.

• Provides social work services in accordance with the plan of care.
• Ensures that changes in the patient's clinical status are communicated to other healthcare team members and the care center office staff in a timely manner.
• Educates the patient and family members and other team members on medical social work interventions and treatment plan.
• Performs care planning, transfer and discharge functions.
• Identifies risks or threats to the patient's health in the patient's environment and promptly implements appropriate interventions to reduce risks. Communicates identification of risks/ threats to the patient to the physician and other members of the healthcare team to coordinate care effectively.
• Other duties as assigned.

Qualifications

• Holds a masters or doctorate degree from a school of social work, accredited by the Council on Social Work Education
• Has one year of social work experience in a health care setting.
• Meets state and federal requirements for this position.
• Current CPR certification.
