Job summary

AMEDISYS HOME HEALTH is seeking a PRN Medical Social Worker to support patients in their homes, helping families cope with health, social and emotional challenges. You will deliver social work services per the care plan and coordinate with the healthcare team to adjust care as patients’ conditions change.

Qualified candidates hold a Master’s or Doctorate in Social Work, have at least one year of health care social work experience, CPR certification, and meet state and federal requirements.

• One year of social work experience in a health care setting.
• Current CPR certification.
• Meets state and federal requirements for this position.

Responsibilities

• Assists the patient and family with health related financial, social and emotional concerns and assists with effective coping skills.
• Provides social work services in accordance with the plan of care.
• Ensures changes in the patient's clinical status are communicated to the healthcare team and care center staff in a timely manner.
• Educates patient and family and other team members on medical social work interventions and treatment plan.
• Performs care planning, transfer and discharge functions.
• Identifies risks in the patient environment and implements interventions to reduce risks, communicating with the physician and team.
• Other duties as assigned.

Communicates identification of risks/ threats to the patient to the physician and other members of the healthcare team to coordinate care effectively.

Qualifications

• Holds a masters or doctorate degree from a school of social work, accredited by the Council on Social Work Education
• Has one year of social work experience in a health care setting.
• Met state and federal requirements for this position.
• Current CPR certification.
