The MSW-Social Worker will effectively communicate and collaborate with all members of the healthcare team to facilitate patient care, ensuring services are delivered safely and promptly. The role involves interpreting and communicating complex patient and family needs, modeling caring practices, assessing psychosocial risk factors, and providing emotional, social, and financial counseling. The social worker will access and mobilize community resources, conduct mental health assessments upon physician referral, and provide therapeutic interventions for discharge planning. Additionally, the role includes serving as a liaison for child abuse/neglect, elder abuse, domestic violence, and guardianship cases, as well as intervening in foster care, adoption, and mental health placements.
