Overview

The MSW-Social Worker will effectively communicate and collaborate with all members of the healthcare team to facilitate patient care, ensuring services are delivered safely and promptly. The role involves interpreting and communicating complex patient and family needs, modeling caring practices, assessing psychosocial risk factors, and providing emotional, social, and financial counseling. The social worker will access and mobilize community resources, conduct mental health assessments upon physician referral, and provide therapeutic interventions for discharge planning. Additionally, the role includes serving as a liaison for child abuse/neglect, elder abuse, domestic violence, and guardianship cases, as well as intervening in foster care, adoption, and mental health placements.

Minimum Qualifications
  1. Master's Degree in Social Work required.
  2. De-escalation certification preferred.
  3. Permanent LSW licensure required; LCSW licensure preferred. Temporary LSW licensure considered with the expectation of obtaining full licensure within the first year.
  4. Relevant healthcare, mental health, or community-based experience required.
  5. Knowledge of disease processes, human behavior, and legal mandates related to child and adult protection.
  6. Understanding of growth and development principles across the lifespan.
  7. Proficiency in crisis intervention and de-escalation techniques.
  8. Ability to develop and maintain strong community and inter-agency relationships.
  9. Proficiency in Microsoft Excel and Word.
  10. 0-3 years of medical social work experience required.
