Location: WI: WEST BEND at Froedtert West Bend Hospital

This is an ON-SITE position.

FTE: 0.001

Shift: 1

Schedule: Part-time, with flexible hours 8am-4pm, Monday to Friday, including weekend rotation (Saturday in person, Sunday pager coverage).

Job Summary:

Under the supervision of the Case Management Supervisor, the Social Worker manages patient and family care during hospitalization, coordinating discharge plans from pre-admission to follow-up. Responsibilities include psychosocial assessments, developing care plans that maximize patient and family benefits, and connecting patients with necessary resources to ensure safe and effective care. Additional duties as assigned.

Experience:

Preferred 1-2 years in an acute care setting.

Education:

Master's degree in Social Work is required.

Skills:

Effective team collaboration, interaction with healthcare professionals and external agencies, strong computer skills, experience in medical social work and discharge planning preferred. Excellent interpersonal and analytical skills are necessary to assess needs, develop discharge plans, and solve related problems.

Licensure:

Current Wisconsin Social Work license required.
