Ardent Health Services in Tyler, TX is seeking a Day-shift, Full-time Social Worker (MSW) Case Manager to coordinate patient care and advocate for quality outcomes.

The role partners with an interdisciplinary team to assess, plan, implement, and evaluate services, provide therapeutic counseling to patients and families, and support resource utilization in a high acuity hospital setting.

Texas licensure or eligibility within 6 months is required, with 1–3 years of acute care experience preferred.

Formación

• MSW required from an accredited program.
• Licensure as a social worker in Texas or eligibility within 6 months of hire.

Responsabilidades

• Coordinating the care and service of patients, promoting quality care, effective utilization of resources, and leading the interdisciplinary team to achieve goals.
• Assessing, planning, implementing, and evaluating the delivery of services in collaboration with the interdisciplinary team.
• Providing therapeutic counseling for patients, families, and interdisciplinary team members to enhance coping and functional skills.

Master's Degree in Social Work

Licensed by the State of Texas or eligible within 6 months of hire

• UT Health Tyler is the flagship hospital of the system serving hundreds of thousands annually. Specializing in advanced acute care medicine, our midtown location boasts a Level 1 trauma center and comprehensive stroke center, the region’s only EMCO program, and a Level III NICU. Our medical staff includes more than 500 physicians and a full complement of subspecialty groups.

Responsibilities

• Coordinating the care and service of the patients, promoting quality care, effective utilization of resources, and assuming a leadership role with the interdisciplinary team to achieve desired goals and resource outcomes.
• Assessing, planning, implementing, and evaluating the delivery of services and working collaboratively with all members of the interdisciplinary team.
• Providing therapeutic counseling for patients, families/significant others, as well as members of the interdisciplinary team to enhance coping and functional skills.

Qualifications

Job Requirements

• Master's Degree in Social Work.
• Licensed by the State of Texas, or obtain licensure within 6 months of hire.

Preferred Job Requirements

• 1-3 years of acute care experience.
