Social Worker MSW
SPPA Monroe

Our QIP-NJ team located at How Lane is looking for an autonomous Social Worker to join our team! This Social Worker will provide care coordination and clinical social work services to our patients in the behavioral health and women's health. The ideal candidate for this position will have previous experience with different age groups and populations and will be ready to jump right in to provide the best care in our busy outpatient facility.

The Social Worker MSW will:

• Round with interdisciplinary team members for the purpose of progression of care, consultation, collaboration and identification of patients requiring further evaluation, development or complex psychosocial issues.
• Conduct coordination of care and document clearly, concisely, and relevantly.
• Work with the Care Transitions Nurse, patient, and/or formal/informal support system to develop and/or contribute to a care plan appropriate for the patient.
• Provide Social Work intervention when appropriate.
• Educate patients and family members.
• Maintain effective collaborative communication with interdisciplinary team members.
• Develop and maintain community relations.

Requirements:

• MSW from accredited Graduate School of Social Work. NJ Social Work License: LCSW or LSW.
• NPI and Medical Non Billing Provider Number are required; the application should be submitted to the State within the first 30 days of employment.
• Previous hospital experience preferred.
• Experience in Geriatrics healthcare.
• Position includes some weekend and holiday coverage.
• EARC-PAS certified within 3 months of hire when applicable to assignment.
