Agape Care Group is seeking a hospice social worker in Meridian, MS. You will conduct psychosocial assessments and provide ongoing support to patients and families navigating end-of-life care.

The role requires an MSW, state licensure, and at least 2 years of clinical experience in healthcare or hospice. Reliable transportation and physical ability are expected as part of field duties.

• MSW from accredited program per CSWE
• Licensed as a social worker in the state
• 2+ years clinical experience in healthcare or hospice
• Reliable transportation and physical ability to lift 25 lbs

Responsibilities

• Conduct psychosocial evaluations for patients and families.
• Provide ongoing counseling and support throughout end-of-life journey.
• Collaborate with the care team to implement the plan of care.
• Assist families with coping, resources, and transition planning.

Qualifications

• A heart to serve patients and families and a passion for providing the best possible care
• Education: MS degree in social work from an accredited school of social work approved by the Council of Social Work Education
• Licensure: Current state license as a social worker
• Experience: 2+ years of clinical work experience, preferably in healthcare or hospice
• Required: Reliable transportation. Ability to sit, stand, bend, move intermittently and lift at least 25lbs and bear the weight of an average adult effectively.
